1. What is ARBESTA (ibertesan, Avapro) and what is it used for?

ARBESTA (ibertesan, Avapro) contains 150 mg of irbesartan active substance. It belongs to a group of blood pressure lowering drugs known as angiotensin II receptor antagonists. Angiotensin-II is a substance produced in the body. It binds to receptors found in blood vessels, leading to narrowing of blood vessels. When the blood vessels narrow, the pressure of the blood circulating in them increases. ARBESTA (ibertesan, Avapro) prevents this substance called angiotensin-II from binding to receptors. In this way, it relaxes blood vessels and lowers blood pressure. ARBESTA (ibertesan, Avapro) slows the decrease in kidney function in patients with high blood pressure and type 2 diabetes (diabetes).

It is available in blister packs containing 28 and 90 film-coated tablets. There is also ARBESTA (ibertesan, Avapro) 300 mg film-coated tablet form on the market.

Your doctor may have prescribed you ARBESTA (ibertesan, Avapro) for one or more of the following reasons:

- Treatment of high blood pressure (essential hypertension)

- In the protection of the kidneys in patients with high blood pressure, type 2 diabetes (diabetes) and impaired kidney function



2. How to use ARBESTA (ibertesan, Avapro)?



Instructions for proper use and dose/frequency of administration

Your doctor will tell you how and at what dose you should use your medicine.

High blood pressure patients:

Your treatment usually starts with a dose of 150 mg ARBESTA (ibertesan, Avapro) once daily (1 tablet of ARBESTA (ibertesan, Avapro) 150 mg). The 24-hour blood pressure control achieved with 150 mg once daily is better than the 75 mg dose. However, initiation of therapy with a dose of 75 mg should be considered, particularly in patients on blood dialysis and in patients over 75 years of age.

In patients whose blood pressure is not adequately controlled with 150 mg once daily, the dose may be increased to 300 mg.

Patients with kidney failure accompanying high blood pressure and type II diabetes mellitus:

Treatment is initiated with 150 mg of ARBESTA (ibertesan, Avapro) once daily, followed by 300 mg once daily as a follow-up dose.
Application route and method

ARBESTA (ibertesan, Avapro) is used orally. Swallow the tablets with a sufficient amount of liquid (eg a glass of water). You can take the tablets on an empty or full stomach. Take your medicine at the same time every day. Taking your tablets at the same time each day will give you the best effect on your blood pressure. It will also help you remember when to take the tablets.

Unless your doctor recommends otherwise follow these instructions.
Use in the elderly

Although it is considered to start with a dose of 75 mg in patients over 75 years of age, no dose adjustment is required in elderly patients.
Special use cases

Renal insufficiency: No dosage adjustment is required if you have renal impairment; however, treatment is started with a low dose of 75 mg ARBESTA per day (you need to take ARBESTA (ibertesan, Avapro) 75 mg once daily).

Your doctor may recommend a low dose at the beginning of the treatment, especially in patients with blood dialysis (a process of cleaning the blood) or in patients over 75 years of age.

Hepatic impairment: No dosage adjustment is required in patients with mild to moderate hepatic impairment. There is insufficient experience in patients with severe hepatic impairment.
Lack of intravascular volume:

Fluid and/or sodium loss should be corrected before starting ARBESTA (ibertesan, Avapro) therapy.

If you have the impression that the effect of ARBESTA (ibertesan, Avapro) is too strong or too weak, talk to your doctor or pharmacist.

If you take more ARBESTA (ibertesan, Avapro) than you should

In case of excessive use, it is estimated that the most common symptoms will be low blood pressure and accelerated heart rate. A slowing of the heartbeat may also be seen. It is recommended to induce vomiting and/or gastric lavage.

If you have used ARBESTA (ibertesan, Avapro) more than you should, talk to a doctor or pharmacist immediately.



3. What are the possible side effects?



Like all medicines, ARBESTA may have side effects in people who are sensitive to its ingredients.

If any of the following occur, stop using ARBESTA (ibertesan, Avapro) and IMMEDIATELY inform your doctor or go to the nearest hospital emergency department:

Sudden wheezing, chest pain, shortness of breath or difficulty breathing

Swelling of the eyelids, face or lips

Swelling of the tongue and throat causing great difficulty in breathing

Severe skin reactions including extensive skin rash, urticaria

These are all very serious side effects. If you have one of these, you may need emergency medical attention or hospitalization. All of these very serious side effects are very rare.
