What is CLONEX and what is it used for?

CLONEX is a drug used in treatment-resistant schizophrenia patients or in patients with schizophrenia who do not respond to or are not tolerated by these drugs.

CLONEX is also used in patients with schizophrenia who are suffering from recurrent suicidal behavior or who are suspected of having a risk of suicidal behavior (schizophrenia or a psychiatric disorder with symptoms of mood swings).

CLONEX is also used in mental illness (psychosis) where the sensation of reality that occurs during Parkinson's disease disappears when standard care fails.

How to use CLONEX?
Instructions for proper use and dose / application frequency

CLONEX is regulated according to your wishes. The lowest effective dose is used for each patient. During the first 18 weeks of the disease, an annual white blood cell count will be made every 4 weeks.

Application path and method

For oral administration the following dosage scheme is recommended:

Treatment-resistant schizophrenia patients

Initial treatment: 1 or 2 tablets of 25 mg on the first day and 25 mg of tablets 1 or 2 times on the second day. If this tolerance is favorable, the daily dose is increased slowly from 25 mg to 50 mg to reach 300 mg per day in 2-3 weeks. Thereafter, if necessary, increases of 50 mg to 100 mg may be made between the daily dose, half-weekly or preferably weekly intervals.

Use in children

The use of Clonex in children under 16 years is not recommended due to insufficient data on safety and efficacy.

Use in the elderly

It is recommended that the treatment be started initially with low doses (12.5 mg one time on the first day) and limit the subsequent dose increase to 25 mg per day.

Special use cases

It should not be used in cases of severe liver, kidney and heart failure. Patients who have previously had permanent liver impairment may use Clonex, but regular liver tests are required.

• Reduced white blood cells (granulocytopenia) or complete disappearance (agranulocytosis) due to bone marrow diseases

• Medication or sickness that can not be controlled by drugs

• Serious mental illnesses related to alcohol or poisoning (alcohol psychosis or toxic psychosis), drug poisoning and coma

• Sudden impairment of the central nervous system and / or circulatory system whatever the cause

• Severe liver, kidney or heart failure

• In the past, when white blood cells are completely destroyed by this drug

• Follicular intestinal obstruction (paralytic ileus)

• Clonex should not be used with drugs that are likely to completely destroy white blood cells.

Use CLONEX CAREFULLY in the following situations

During the use of Clonex, there is the possibility of a subsequent reduction in white blood cells that may result in complete disappearance. For this reason, your physician will ask you regularly to have a white blood cell (white blood cell) count.

If symptoms such as fever or sore throat are seen indicating the presence of a flu or flu-like condition, you should consult your physician without delay. During clonex treatment, blood pressure reduction may occur. If you have a complaint like dizziness, you should consult your doctor.

Especially in the first weeks of treatment, Clonex will cause drowsiness and it will make it easier to have epileptic seizures and avoid using the car or machine.

Clonex suffers from sleep deprivation and fatigue, so be careful not to be immobilized.

During clonex treatment, body temperature may rise above 38 ° C in some patients. Although this fever is usually not important, you should tell your doctor. In the presence of high fever, the nervous system may develop a serious disease called Neuroleptic Malign Syndrome (NMS).

• For these reasons, Clonex treatment should be started under medical supervision. You should consult your physician to investigate if you have complaints such as persistent palpitations, heart rate irregularity, chest pain, unexplained fatigue, shortness of breath, frequent respiration especially during the first two months of treatment. Such a situation can be very serious, so the medicine may need to be discontinued.

• If you are a Parkinson's patient at the same time, you should monitor the standing and resting blood pressure during the first weeks of treatment.

• Patients who have previously had permanent liver impairment may use Clonex, but regular liver tests are required. Liver tests should be performed in patients with evidence of possible liver impairment such as nausea, vomiting and / or loss of appetite during Clonex treatment. Depending on the test results or if there is a jaundice, the Clonex treatment should be discontinued. Treatment should be continued only when the liver tests are normalized. In such cases, the condition of the liver should be closely monitored after the medication is restarted.

• Prostate enlargement or some eye diseases require special care.

• Clonex treatment can be accompanied by various severity of disorders ranging from cavitary to bowel obstruction in bowel movements. These rarely result in death. Particular attention should be given to patients who have received medications known to cause constipation, who have undergone a lower abdominal operation that may exacerbate or develop a large bowel condition in the past. It is very important to diagnose constipation immediately and to treat it effectively.

• During treatment with clonex, severe blood sugar elevation has been reported rarely, sometimes coma leading to patients who have not previously had high blood sugar levels. It should be remembered that patients with clozapine are at increased risk of diabetes when symptoms include frequent eating, frequent drinking and frequent urination or weakness. Stopping Clonex therapy should be considered if this treatment is not medically effective.

Use in the elderly

Complaints such as low blood pressure and palpitation during clonex treatment may be more common in elderly patients, especially those with cardiovascular problems.

Elderly patients may be more susceptible to Clonex's effects, such as urine accumulation and constipation.

As this group is used in elderly patients with dementia, as it is in other medicines, it is likely to cause an increased risk of death due to cardiovascular diseases, inflammatory diseases, heart failure, etc.
